Disston Community School, located in the Tacony neighborhood of Philadelphia, is a Philadelphia public, K-8 school selected by the City of Philadelphia for Community School designation and supports. CORA Services is searching for a Case Manager to play an integral part of the Community Schools team, committed to serving, supporting and strengthening the Disston community. The Case Manager will work collaboratively with schools, students and families to provide supports to students and families experiencing socioeconomic or health stressors. The Case Manager will carry a caseload and work to help families increase protective factors, increase family functioning, increase family engagement in school activities and increase student's attitudinal connectivity and relationship to school, which, in turn, will increase student attendance and achievement. The Case Manager will work primarily from the school, but will also be responsible for conducting regular home visits and supporting families within the community. In addition, the Case Manager will assist the team with the coordination and implementation of monthly family forums.


The Community School Case Manager is passionate and committed to making a difference in the lives of youth and families in the Tacony neighborhood. The Case Manager is skilled in conflict resolution, team building and engagement, and is able to work collaboratively with team members, school staff, partners and families. The Case Manager is resourceful, caring and committed to impacting families and the school community



Community School Case Manager Salary: $42,000 (based on exp.)

Community School Case Manager Qualified candidates should possess the following:

  • Bachelor's degree in Social Work or related field
  • Two or more years of experience in social services, specifically with a focus on serving children and families
  • Demonstrated ability in supervision/administration
  • Valid driver's license
  • Reside in zip code 19135 or have a strong familiarity with the community - including its resources, assets and needs
